Has openings in its Day Hab & Pre Voc Programs as well as Supported Employment
Life Adjustment Center:
Has openings in their Northern Harlem location for adults who are Medically Frail and Ambulatory.
Has openings in our lower Manhattan Location for individuals who are Diagnosed with Mild Intellectual Disability
Re Cap on Student Information Exchange:
Everyone seemed pleased with the student information exchange and had only one recommendation for the future: Microphone should be made available from the beginning.
It was stated that the Student Information Exchange was easy to navigate.
It appeared as though more referral packets went out this year, even though they may not have had all evaluations included.
Status of 2012 Graduates: The committee reviewed the referral/placement status of this year’s graduates:
PS138: Has a total of 16 graduates – 5 have Day Hab Placements lined up.
PS751: Has a total of 40 graduates; most of them are going to ACCESS –VR; No anticipated problems
PS226: Has a total of 14 graduates – 7 have placements lined up
PS721: Has 34 Graduates – 11 have been referred to ACCESS-VR with the remaining students divided between Day Hab & Supported Employment
PS 79: Has 36 graduates – 5 have Day hab Placements lined up; about 10-12 are referred for Supported Employment; the remaining are referred for Day Hab of which 3-4 are Medically Frail.
PS226: Has 14 graduates – 7 have Day Hab placements lined up
Cooke Center: Has approximately 25 graduates of which ½ are seeking OPWDD services. These are split between Day Hab Without Walls & Supported Employment.
