Student Information Exchange

Kathy Kelly opened the meeting with an overview of the Manhattan Transition Committee which is a sub-committee of the Manhattan Developmental Disabilities Council. The main focus of the TC is to assist students leaving the Department of Education transition into adult services. Kathy emphasized that all parents and agency and Department of Education representatives are welcome to attend the Transition Meetings held on the second Wednesday of the month at UCP-NYC, 122 East 23rd St. If you would like more information about our committee please contact either Kathy Kelly or Kathy Beiser at the numbers listed at the end of these minutes.

Kathy outlined the work and program opportunities available for students once they leave the Department of Education. They are(listed in order of amount of supported provided, starting with those with the least support and ending with those with the most): Competitive Employment, Supported Employment, Pre-vocational and Work Readiness, Day Habilitation and Day Treatment. Kathy also explained Medicaid Service Coordination (MSC) which is not an adult service but a Medicaid support service for all individuals (children and adults) with a developmental disability. MSC is required for entry into most of the adult programs.

Each attending agency then gave a brief description of the programs they offer. This was followed by the exchange of information between transition coordinators, parents, students and the representatives from the adult agencies. We were pleased that so many students and families attended and were able to meet with representatives from various agencies.
