Soho Day Habilitation-Lower Manhattan
Individuals must be diagnosed with an Intellectual Disability

OPWDD:
Tom Mannion clarified from the last meeting the information that he needs to gather
He has to collect data on the individuals that aged out in the past year who haven’t
been placed and he also needs to collect data on those individuals who are currently
looking for placement. Data on this information can be sent to Tom via email at:

Open School Night:

PM721M is hosting its open school night on 11/7/2012 and there are two sessions the
first one is during the day from 12:00pm to 3:00pm and the second session will be at
night from 5:00-7:30

Manhattan Family Support and Resource Fair:
Will be held on November 29th, 2012


DD Council and the Manhattan Family Support:

Will be holding a joint meeting on how to advocate effectively with Legislators.
No RSVP’s are needed
This meeting will be held on October 30th, 2012 form 5:30pm to 7:00pm at YAI
YAI is located at 460 West 34th Street (12th floor; J and J1)

Intensive Behavioral Supports:
Intensive behavioral supports are available for those individuals with severe challenging
behaviors. These supports are given in two phases. In the first phase the individual
will have evaluations in which a plan is developed in how to work with the
individual. The second phase will be training in how to implement the plan

Review/Update of Adult Day Services Directory:

Supportive Employment has been changed to ACCESS VR, formerly known as VESID

Work Readiness is teaching an individual the basic understanding of work
For Example: teaching the individual how to fill out an application

E-SEMP is designed to allow flexibility in development of the individual employment outcomes and affords individuals with significant barriers the opportunity to work in the community. Time and duration of staff support is flexible

Discussion was held as to if ESEMP and Supportive Employment should be listed separately in the Manhattan Adult Day Services Directory and a new section will be incorporated into the directory.
