OPWDD Front Door Initiative:
What is the Front Door Initiative – The Front Door Initiative is OPWDDs new way to centralize the entry and access to services for individuals with DD. It is part of the transition into managed care & is very closely tied to the service transformation agreement made with CMS. OPWDD has begun this initiative effective June 1st with a “soft launch.” They are starting this initiative with individuals new to OPWDD services and those individuals seeking additional services.
Some Main goals of the Front Door Initiative:
1. Ensure consistency & equity of services offered to individuals/families
2. Promote Self-Direction/Self Determination
3. Promote Employment as a service option
4. Build on the philosophy of Person Centered Planning to ensure that individuals/families are given a true informed choice of the full array of service options
The Front Door Process:
1. Initial Contact
2. Eligibility Determination
3. Assessment
4. Identification of Service Needs/Plan Development
5. Service Authorization
6. Service Implementation
What are the impacts today? – OPWDD is currently requesting that individuals transitioning out of school and requesting OPWDD Day Services be referred to the DDROs to participate in this initiative. The contacts are as follows:
