The focus of this meeting was to prepare for the Student Information Exchange scheduled for 3/12/14. The Student Information Exchange is an annual event hosted by the Transition Committee to facilitate referrals to Adult Day Services. It will be held at P79M this year.
Representatives from the schools should invite students and families to come and meet the adult day service providers. All Adult Day Service Providers are invited to come and briefly present an overview of their services. There will also be an opportunity for school staff, students and families to talk more individually with the adult provider representatives.
School Transition staff should prepare referral packages for the students to include the following:
- Psychological Evaluation
- Psycho-social evaluation
- Medical/Physical Exam
- IEP
- ISP – If the student has MSC already
Narrative summary of what the student is looking for along with their strengths/interests and needs – including any special accomodations (i.e. wheelchair accessibility, significant behavioral considerations).
It is also important to keep in mind that the Front Door Process needs to be completed for each student graduating this June.
