AHRC, New York City:

AHRC has Day hab openings at their Varick St,& 315 Hudson St locations Openings in Supported Employment & the Hotel Housekeeping training program.

General Human Outreach:

Has opportunity’s in Pathways to Employment as well as Intensive SEMP

Additional opportunities are available in Day Hab as well as MSC with languages in Mandarin as well as in Cantonese.

Human First Inc

There are openings in Day Hab, SEMP, and Pathway to Employment, as well as MSC.

                

YAI:

Has openings in Supported Employment & Community Based PreVoc.

 

Catholic Guardian Services:

Catholic Guardian Services has a new Community Resource and Transition Center to educate people with Developmental Disabilities and their families on the supports & services available and to assist in accessing needed services. See Attached Flyer for more info.

 

Manhattan Family and Resource Fair:

Will be December 14th at the NY Academy of Medicine located at 103rd and 5th.

 

June 2016Graduates:

At this time the schools project approximately 90 Manhattan students expecting to graduate in June 2016 who will be transitioning into OPWDD Adult Services. The school staff reported that the vast majority have already been determined to meet OPWDD eligibility and over half have MSC in place. The school transition representatives are actively working with the Metro DDRO’s staff to coordinate & ensure the process is underway. All graduates need to go through the Front Door. It’s important to remember that all graduates need to complete the full Front Door process and that although the attendance at a Front Door Information Session is required; it is not the only piece requiring the full participation of the individual/caregiver. Some families have misunderstood that once they attend the information session, they have completed the process.

 

Open Discussion on Improving the Transition Process:

The committee is looking to improve the Transition Process by trying to identify ways to increase and improve the information and outreach to students/families at an earlier age. An open discussion was had and some suggestions including more collaboration with Parent Coordinators at the schools, as well as with Parent/Teacher conference dates. It was suggested that family outreach at the time that the student is entering the high school, generally at age 14 could be a good time since students/families are usually more involved since it’s a new environment.
