Manhattan Family Support Fair:
December 11th from 9:30 to 2:30 @ Hotel Pennsylvania and is sponsored by the Manhattan Transitional Committee thus our Committee is responsible for the food, set up, and clean up. Therefore, volunteers from our Committee are needed. Please Contact: Kathy Kelly if interested
Pre- Registration is needed for school groups
Transition Steps needed for 2014 Graduates:
1. Schools should send list of students’ names & identifying information to Evette Herdsman at Metro DDRO (212 229-3037). Evette will check OPWDD info to determine if the student is known to OPWDD and if eligibility for services has been established.
2. Current evaluations as per OPWDD eligibility guidelines will be needed for those with no eligibility determination on record.
3. For those students currently enrolled in a waiver service, no Front Door is needed at this time, however participation at a Front Door Information Session is optional.
4. For those students not currently enrolled in a waiver service, attendance at a Front Door Information Session is required. There is one scheduled at P226M on November 27th. There will also be Front Door Information Sessions at the Manhattan Family & Professional Resource Fair on Dec. 11th (see attached brochure). Information was also given on how to register through OPWDD’s website.
Student Information Exchange:
The Student Information Exchange is a large scale Transition Meeting where All Adult Day Service Providers come together with the schools to so that referral packets can be given out to the appropriate provider. Prior to the Student Information Exchange tours of the provider agencies should be conducted, with the families to help determine which are the appropriate providers. Included in the packets are the Evaluations, Medical, PPD, and a Snap Shot Profile of the individual. If the completed packet isn’t ready any pertinent information that can be gathered should be included.
The Student Information Exchange will be March 2014. Date, Place, and Time are to be determined the space for the Exchange has not yet been found. The Committee needs space large enough to hold about 100 people. A school gymnasium, large conference room to hold several different tables, etc. would be ideal.
