Subject: Residential Request List Survey
The following message was sent to all stakeholders registered to receive OPWDD updates. Since not everyone receiving services has registered to receive agency updates and we want to make sure this message reaches everyone, please share the message below with individuals or family members that you know are interested in receiving residential services. Also, please encourage people receiving services and their family members to sign up on the home page of the OPWDD website<http://www.opwdd.ny.gov/index.php> to receive important agency messages and updates:
In August, OPWDD began surveying families and individuals on the Residential Request List (RRL) about the services they are currently receiving and those which they may be interested in receiving. During these calls, we pay special attention to the residential supports that an individual and family may need now or in the future. The information we gather from the survey will not be used to change an individual’s service plan but rather to update OPWDD’s Residential Request List, previously known as the New York State Cares registry, and to help plan for future residential services that individuals and families need. Individuals and families should continue to work with their MSC/Service Coordinator and the OPWDD Regional Office to plan for specific needs.
Anyone with an interest in residential supports now or in the future who has not yet participated in the survey is encouraged to complete a survey prior to 5 pm on Saturday, October 10, 2015 by calling 1-844-679-3369, Monday through Friday between 5pm and 8pm and Saturday between 9am and 5pm. You do not have to currently be on the Residential Request List to complete a survey. As part of the survey, the call representative will ask the following questions:
– the name and date of birth of the individual with developmental disabilities
– if the individual/family is still interested in residential services,
– Where the person is living now,
– Who he/she is living with,
– Who is helping him/her,
– How old the helper or caregiver is and if the helper or caregiver has any issues that make it difficult to do everything that the person needs,
– Whether the person with developmental disabilities is getting the right OPWDD services to support him/her where he/she is living now, and
– What services the person with developmental disabilities will need in the future.
