Eligibility for Nicosia Family Reimbursement
- There must be a family member with a developmental disability. Single individual with a disability living alone are not eligible.
- Families receiving services from the foster care system are not eligible.
- The requested support must be appropriate for the person with a developmentally disability and other sources of support (Medicaid, insurance, bank, loans) should be pursued first without success.
- The requested support should not be normally funded through other sources (Medicaid, insurance). For example, if you are applying for reimbursement of a Doctor bill, you must include a letter from the doctor stating that insurance/Medicaid did not pay for the service and why.
- In most circumstances, the applicant cannot have been awarded in the previous or current fiscal year. The fiscal year starts July 1st.
Application Process
- The application should be submitted complete and with all the required information. If we get applications that are incomplete, we will send the entire application back with the request for the completed material. We will not hold partial applications. The applicant must re-submit the whole application package in order to be considered for an award. In addition, all receipts need to be from July 1, 2013 to June 30, 2014.
- Once the application is complete, the application will be logged into our system in order received.
- The committee will meet at least once every quarter to review applications in order received and will allocate money according to the standard criteria of the program.
- The committee will first consider whether the person was awarded in the past year, the income of the family and the family size. Next, they will consider whether the items or services requested will enhance the quality of life of the consumer with a disability. The committee will also look at any special circumstances in the case. Please thoroughly explain in the application or a cover letter why the item or service is needed and how it will assist the person with a disability. Include any recommendations from doctors or therapists to support the application.

- Family Reimbursement

Provides limited funds to families for services and goods that are not reimbursed through other existing programs or funding streams.
- Emergency Respite Fund
(Brooklyn, Manhattan & Queens)
Funds are available to families to help provide temporary caregiving during extreme family emergencies and to pay for staffing, food, and other necessities in accordance with the unique needs of each family.
- Free Psychological and Psychosocial Evaluations
(Brooklyn, Queens & Manhattan)
Free evaluations provided for individuals who do not have Medicaid but require psychological and/ or psychosocial to access programs for people with developmental disabilities.

- Transportation Reimbursement
The Transportation Reimbursement Program is funded by the Family Support Services of the New York State Office for People with Developmental Disabilities to reimburse Manhattan families who have a person with developmental disabilities living at home with them.
Requirements:
- Completed Registration Form
- A Copy of the Current Psychological Evaluation
- Monthly Expense Submission Form
Reimbursement will be made up to: $75.00 per month.
Transportation reimbursement checks will be sent out once a month.

- Family Reimbursement
The program is limited to families living in Manhattan, above 110th Street.
The following items must be completed and submitted on a one time basis, before or along with the first application:
- Registration form
- Financial status form
- Current Psychological evaluation which must include all adaptive behavior and full IQ scores

According to the DDRO, we were instructed that new people requesting OPWDD services are required to go through The Front Door. New people are defined as
- Individuals whose eligibility has not yet been determined
- Individuals who have been have determined eligible for OPWDD services but never received OPWDD services or have not enrolled in an active OPWDD services for over a year
- OPWDD eligible individuals receiving only MSC and/or state funded services.
Individuals who currently receiving MSC together with any OPWDD waiver service, are not required to go through the Front Door.
Please complete the application and attach the following documents:
- Copy of Psychological evaluation.
- Completed DDP1
- Original dated receipts (dated within 7/1/2013-6/30/2014) with Name and Address of the Store/Service provider. Item purchased and amount must be stated clearly on the receipt.

Family Reimbursement is for Manhattan, Brooklyn and Bronx residents only awards are not guaranteed.
The approval process can take 3 to 6 months
Eligibility for UCP of NYC Family Reimbursement
There must be a family member with a developmental disability and the individual must be living with a family member.
According to OPWDD Family Support Services guidelines, it is imperative that all applications contain documentation of a developmental disability. If the individual is already OPWDD eligible a psychological is not needed. But you must submit one of the following:
- OPWDD Eligibility letter.
- MSC determination letter.
- Current Level of Care Form – the LOC cannot be more than a year old.
MANHATTAN residents MUST provide a copy of a psychological report.
The merchandise or service requested must only be for the benefit of the person who has a developmental disability.
Funding available for Durable Medical Equipment for Manhattan Resident
Medical documentation supporting a need for the equipment must be included.
You must show proof that Medicaid or your medical insurance does not cover the item.
FISCAL YEAR:
July 1 to June 30. All receipts must be dated within the current fiscal year. The application and any estimates for goods or services must also be dated within the fiscal year.
ALL APPLICATIONS MUST INCLUDE A DETAILED STATEMENT OF NEED (Why should this family be reimbursed)

Family Services Support Reimbursement Program
In order for your application to be considered by the Parent Advisory Council you must also submit:
- Documentation of a Developmental Disability:
Psychological evaluation (IQ score or an Adaptive Behavior Composite score) and documentation of age of onset prior to 22 years old or OPWDD eligibility determination letter.
- Support letter:
You must include a support letter from your Medicaid Service Coordinator, parent or medical Physician explain why the item/goods/service is needed.
- For Respite Reimbursement:
You must have the Respite form filled out and notarized
- Original Receipts:
Of the items you have purchased. Receipts must be within the fiscal year that starts on July 1, 2013-June 30, 2014.
The Parent Advisory Council will NOT consider applications that are submitted without the above proof of disability. Also, $500 is the maximum amount allowed on reimbursement requests.

Quality Services for the Autism Community (QSAC)
Camp Reimbursement for Manhattan residents
You may receive partial or full reimbursement if your child meets the following criteria:
- Has a diagnosis of Autism
- Lives at home
- Attends a day or overnight camp program

Epilepsy Foundation of Metropolitan New York
Reimbursement funds are available to OPWDD eligible persons with Epilepsy and must be used to help the person obtain/maintain skills fostering independence, productivity, and/or integration within the community. Due to limited remaining funds or committee will provide a maximum of $200 per request. Priority will be given to persons who have exhausted other alternative funding. An application does not guarantee approval and is at the discretion of the reimbursement team.
The application requires documentation of
- Epilepsy
- Documentation of OPWDD eligibility
- A letter outlining the amount of the request and its proposed use
