Manhattan Based recreation Programs

Overnight Respite

Weekend Day and Evening Recreation Program

Manhattan School Holiday Program.
5 day trip to Washington, DC from April 18th – April 22nd. (During NYC BOE Spring Break) the trip is free for students ages 14 – 21 and living with a family.

Jane Hsu, the founder of special playdate just launched this program and it is self funded.
The Special Playdate site brings people together to form a warm and friendly community which helps children with special need. Visitors can receive weekly playdate tips featuring playdate ideas and tips, behavior management techniques, ample opportunities to discuss your experiences on a blog and in forums with other parents. It is a similar process as Match.com and you don’t need to use your real name.
Special Playdate offers a FREE and EASY way to get started. It’s an online service connecting parents with other parents seeking playdate opportunities for their children with special needs. Members are able to securely search for appropriate peers in their local communities and can also use the private message system to connect with other parents before scheduling or revealing personal information. Jane is eager to network with providers and families in New York City and has an educational background in special education.

Metro DDSO, OPWDD
DDP1’s
Are used to add or remove from a program, address change or if someone passes away. TABS are on any document generated by OPWDD. Need to add the program codes on all DDP1’s.
For all Family Support programs you must send a DDP1. If someone moves to another borough, OPWDD needs the new address on record.
DDP4’s (unmet needs)
These are also used for adding someone to the residential waitlist.
DDP2’s
They must be updated every two years. The DDP2’s need the agency code. The person must be already in OPWDD’s system and currently receiving any service.
After you submit a DDP2, you can then get an ISPM score: 1 – 6 (the higher the number, the greater the persons needs).
While anyone can fill out the DDP2, the person who knows the consumer the best should complete the DDP2. It was recommended that the DDP2 and the Psychological evaluation share coherent information about the person.
The code 95 on EMedNY allows you to dis-enroll a person from Medicaid Managed Care. There is no connection to funding based on the DDP2 scores.
You will never get a NOD without a DDP2 being submitted. The DDP4 goes to Melanie Caldwell at the Bronx DDSO.
