The New York Service for the Handicap – Camp Oakhurst – has respite availability for out of home overnight services from September – June. The length of stay is from 5-7 days for adults and 2-5 days for children. Door to door transportation is available, Call 732.531.0215 for more information.
Jacqueline Rumolo from CP of NYS reported that she will have information shortly on a Travel Training initiative through the MTA.
Margaret Puddington noted that there is a change in the language for the Manhattan RFP for Innovative Programs. The language should read:
“We are looking for creative programming! Examples ; Unusual Reimbursement, Sports/Athletic Programs, Inclusive Programs, Vacation/Respite, Sexuality Training, Skills Training, etc. Transportation must be provided as appropriate.”

Lance Galvin, Metro DDSO: Processing the MSC Application
There were 3 handouts that were circulated by Mr. Galvin.

It is recommended that you call the DDSO first to see if a person you are applying for is really in the system.
If a consumer/family wants to select another MSC they need to submit a change of Vendor form.
What to submit for MSC :
MSC Form, a SS number and date of birth
If you submit an MSC application and the person is not known in the system you must submit an entire package for eligibility.
Lance utilizes the CR-4 print out form; a state coded form; and on the CR-4 you have an entire history on the consumer.
MSC-1 Form
Make this as complete as possible. It should be right the first time. They need the
ORIGINAL FORM MSC-1. You are better off mailing the MSC-1 to Lance’s attention. The MSC-1 form must have the signature of the family.
OPWDD cannot back date the effective date.
Please make sure the names are spelled correctly on the forms.
ELI-01 Form (Transmittal Form)
Must have the SS number and Medicaid number
Section 4 can be problematic – check off what is needed at the time. If you just want DD determination only, check one.
A letter will then go goes to the family and the agency. You can ask for a review if you do not agree with the determination.
There is NO CAP on MSC! There are no allotments.
DDP1
Must go with the application – this is mandatory initially.
Manhattan and Bronx (Metro) require the DDP4 with the MSC application.
Please make sure you know your agencies code. The agency code is 5 digits and 3 digits for the agency you are licensed in. Only a few agencies are licensed in all 5 boroughs.
Process:
Approval for MSC eligibility is usually given a response in 2 weeks. If not approved, after eligibility it could be a problem with growth and or proof or Medicaid or there could be an issue with double billing.
We all agreed to be persistent as advocates if you feel strongly that the individual should be eligible for services
Change of Vendor Form
If a person is unhappy with the agencies services and you can not placate the family, we need to assist in completing a Change of Vendor form. The agency submitting the Change of Vendor form is the agency who will be taking the new case.
For a Change of Vendor form they need an MSC-2 Form & DDP-1.
You can fax this information but they prefer a copy through the mail.
Some agencies submit a letter explaining why the change is being submitted and the family should sign it. The NOD will be sent to the previous provider, the new provider and family. This makes the process go faster. When you get the NOD, you can start the service. Signatures are required by the family, person served and the MSC.
With MSC, we need to document that the person served really needs ongoing & comprehensive services.

Secure Messaging System (SMS)
Rachel circulated the applications for the SMS and then instructed they be mailed back tot eh DDSO. When you get on the system for FS programs it takes your FS Rosters and the TABS.
In the future we (agency) will only add people to the system. We need to mail the DDP-1’s to the DDSO and then we get the approval letters. Never remove anyone from the rosters on the SMS.
If the agency does not add a person for service (it cannot be back dated) to the roster, they will not get paid.
On July 1, 2011 OPWDD will require only family reimbursement. Consumers to be deleted then added to the roster.
The rosters must be kept up to date.
OPWDD Family Support Quarterly reports are not done electronically. Some Family Support liaisons require a hard copy of the Quarterly Report.
Parent Satisfaction surveys in Spanish were circulated for Family Support waiver respite programs. These are state issued survey forms that must be used.
