Graduates:
It appears as though most graduates have been placed or are in the process of being placed. The Transition Reps from the schools reported that a few of the 2015 Graduates are still in the process of obtaining services.
All graduates need to go through the Front Door. It’s important to remember that all graduates need to complete the full Front Door process and that although the attendance at a Front Door Information Session is required; it is not the only piece requiring the full participation of the individual/caregiver. Some families have misunderstood that once they attend the information session, they have completed the process. A review of the OPWDD Eligibility Criteria and Front Door Process was provided for any new committee attendees.
Adult Day Service Directory Review:
The Manhattan Adult Day Service Directory was provided so that agency representatives can make any changes. The Directory will be updated and made available before the Manhattan Family & Professional Resource Fair on Dec. 14th. Agencies are asked to return any changes to Kathy Kelly at Kathy.Kelly@ahrcnyc.org.
