Soho Day Hab With -Walls, (in lower Manhattan), for individuals who are mildly to profoundly Mentally Challenged
QSAC:

Catholic Guardian Services: Opening of the Community Resource & Transition Center: The purpose of the center is to educate people with I/DD and their families about the array of supports & services available under the HCBS Waiver and to help individuals access these services. See attached flyer for more information.
Graduates:
The focus of this meeting was to review the progress of securing services and supports for those students graduating in June 2015. Each school present reported on the progress of their students who will be needing OPWDD Adult Supports & Services. A summary of the current status of the 108 students reported on at the meeting is as follows:
40 students pursuing Supported Employment
60 students pursuing Day Habilitation – 32 Authorized with provider selected; 16 currently selecting provider; and 12 in Eligibility/Front Door Process.
Provider Agency’s need to reach out to the Transitional Coordinators as well as the Transitional Coordinators reaching out to the Provider Agency’s. That is: if this hasn’t been done already
