OPWDD:
OPWDD has moved to 25 Beaver Street and is no longer located at 75 Morton Street. Emails and address

The Front Door:
Individuals who are aging out of the Board of Education need to access OPPWD Services via The Front Door. It is important to keep in mind that this process relies on the initiative of the individual/family to participate. The family MUST make that contact with the Front Door staff at the local DDRO and participate in a Family Information Session prior to the student being authorized to receive adult services.
Medicaid:
One must have Medicaid in order to receive most OPWDD services. However, if one is going to Supportive Employment ONLY, one does not need Medicaid.
There are 3 different Levels of Medicaid Insurance:
1) Medicaid with no Community Based Long Term Care which includes:
Clinical Visits, Health Needs, and MSC
2) Medicaid with Community Based Long Term Care which includes:
Long Term Care, Day Habilitation Services, and all Waiver Services, plus everything that
Level One includes
3) Medicaid with Full coverage which includes:
Nursing Homes, Intermediate Care Facilities, and everything that Level One and Two pays for.
The criteria used to differentiate the 3 levels of Medicaid are as follows:
Level 1: Medicaid Application, income, and resources
Level 2: Documentation of current Income and current recourse
Level 3: Documentation of current income and resources going back 5 years
Student Information Exchange:
The Student Information Exchange is a large scale Transition Meeting where All Adult Day Service Providers come together with the schools to so that referral packets can be given out to the appropriate provider. Prior to the Student Information Exchange tours of the provider agencies should be conducted, with the families to help determine which are the appropriate providers. Included in the packets are the Evaluations, Medical, PPD, and a Snap Shot Profile of the individual. If the completed packet isn’t ready any pertinent information that can be gathered should be included in the Evaluations
The Student Information Exchange this year will take place on 3/12 at P79M, the gymnasium. The School is located at 120th and Madison in Manhattan.
