Day Habilitation With Out Walls
Pre Vocational
OPWDD (Office For People With Developmental Disabilities) The Front Door Initiative:
The Front Door Initiative is designed to be a single point of entry for individuals seeking OPWDD services. It began in June 2013 with a “soft launch” focusing on individuals new to OPWDD services. The services received under OPWDD are not an entitlement unlike the services received while under the Board of education auspices. Under OPWDD the services are based on eligibility and need. The following criteria is used to determine eligibility
1) Diagnosis of a Developmental Delay, Intellectual Disability, Cerebral Palsy, Neurological Impairment, or Familial Dysautonomia
2) The diagnosis occurred prior to the age of 22.
3) Substantial Deficits in the area of Life Skills
The Front Door Initiative is for individuals entering our system for the first time. However, eventually everyone, even those individuals who are already receiving services, who want a change will need to go through The Front Door. For those students graduating in June 2014 it is likely that the Front Door Process will be fully implemented and the transition process should take this into account. This initiative is a 6 step process which will be used to determine the following: if individuals will be eligible for receiving our services and the services an individual will be authorized to receive. The process includes:
1) Initial Contact
2) Eligibility
3) Assessment
4) Identification of Service Needs/Plan Development
5) Service Authorization
6) Service Implementation
The Main Goals of The Front Door Initiative are to:
1) Ensure consistency and equity of services offered to individuals and families
2) Promote Self- Direction/Self-Determination
3) Promote Employment as a service option
4) Build on the philosophy of Person Centered Planning to ensure that individuals/families are given a true informed choice of the full array of service option
General Overview of array of services provided under OPWDD:
I. Adult Day Service Options:
Supportive Employment:
Provides assistance and support to help individuals obtain and maintain a job. Individuals that are enrolled in Supportive Employment will have a job coach. An individual needs to be travel trained
Enhanced Supportive Employment
This service has been a pilot project and is in the process of being phased out. This program was for individuals with a Developmental Disability who needed need high levels of support can successfully work in regular jobs in the community. In this program individuals need to find a job within 3 months of enrolling in Enhanced Supportive Employment.
Work Readiness and Pre-Vocational
Work Readiness and Pre-Vocational are essentially the same but are from funded from 2 different funding sources. Work Readiness is funded through a City Contract and Pre-Vocational is funded through Medicaid. These services provide skill development to enable an individual who may not be ready for employment, but need support in obtaining the skills needed for employment. It should also be noted that effective July 1, 2013, new individuals enrolling into Pre-Vocational services need to be delivered in an integrated setting – not in a Sheltered Workshop environment.. Transportation may be available.
Day Habilitation:
Program Day runs generally from 9:00-3:00 and works on goals related to:
Socialization, Adaptive Daily Living Skills, Money Management, Functional Academics, and
Community Inclusion
Transportation is available but is not mandated
Day Habilitation With Out Walls
This is a program where services are delivered in the Community. A staff member may meet the individual at home or in a central location and then bring the individual out into the community to work on goals rather than working on goals at a site such as: A Day Habilitation Program
II. Other common OPWDD Services
Community Habilitation
Supports are provided in the home or the community and can consist of one staff to one individual and it doesn’t need to necessarily take place in a group setting in the community like Day Habilitation With -Out Walls. Community Habilitation can take place at the same time as Home Attendant Services as long as the goals are not the same
Individualized Residential Alternative (IRA)
There are two kinds of IRA’s: Supervised IRA and a Supportive IRA. The difference between the two is that the Supervised IRA’s provide 24-hour staff support and supervision in a certified residence, whereas Supportive IRAs provide less than 24 hr. oversight.
