- Isaacson Gordon Lodge
A sleep away camp, 2 and ½ hours outside the city, and is for Developmentally Disabled Individuals who require no more than a 1:4 ratio. There is a session specifically for teens targeting ages 13 -17 from Aug 17-27th. There is transportation to and from camp. There will be an open house the weekend of June 16th , and if you can’t make the open house there are videos of camp available on the website. This camp offers your traditional camp experience that includes but is not limited to: Music, Drama, Swimming etc.
AHRC Fisher and William St:
Has openings in both of its Day Habs
- Fisher Day Program has about 6 out of 7 openings
- William has about 2 to 3 openings.
AHRC NY League:
Has openings in its Day Hab and Pre Vocational Programs
GHO:
Has openings in most of its programs
- Day Hab-transportation is provided
- Pre Voc- bilingual Cantonese
- MSC
- Family Support for Non Medicaid individuals
Life Adjustment Center:
Has openings in all of its day programs
- Bronx Day Program – high functioning individuals diagnosed with Intellectual Disabilities
- Lenox Day Program- low functioning individuals who are medically fraile but ambulatory
- Soho Day Program- high functioning individuals diagnosed with Intellectual Disabilities
- Empire Day Program- Individuals with Autism or along the spectrum
YAI:
Has no openings in its work readiness program
People First Waiver:
A new waiver which, by 2014 all Medicaid services will need to be incorporated into a
Managed Care Program
- An assessment tool will be used for each individual to assess their level and need for service
- This Waiver program will work in terms of dollar amounts that will go towards the supportsManhattan Transition Committee Meeting

Epic Annual Fair:
Was held on May 19th, 2012 at 383 Adams Street
- Workshops were given for families
- Lunch was provided
District 75:
Has a new exhibit at the main site, 400 1st Ave, which is a timeline of our education system supporting individuals with disabilities. The exhibit is free
Letter Of Intent:
OPWDD issued a call for Letter of Intent for agencies seeking to provide work and meaningful activities to individuals transitioning out of education services.
Status of 2012 Graduates: The committee reviewed the referral/placement status of this year’ graduates:
P 138: Has a total of 16 graduates- 5 have Day Placements lined up
P 751: Has a total of 40 graduates-most of them are going to ACCESS VR; no anticipated
problems.
P 226: Has a total of 26 graduates- 15 to 16 already have placements lined up
P 79: Has a total of 37 graduates- 10 of which are already placed, 1 individual refused services, 2 individuals have Adaptive scores that are too high, 1individual’s Medicaid was denied, 23 are going to various agency’s.
