Free Standing Respite Openings are available One doesn’t need to be waiver enrolled Funding is provided via Family Support Funding Medication is administered in this program Recreation is a big focus 1:1 can be provided if need be There are no limits for clients to attend this program except for the fact that they need to be ambulatory
AHRC Fisher Day Program and Williams Day Program Openings are in both Day Programs AHRC Harriman Lodge in the Catskills Openings are available
GHO Openings are available in the following programs Pre Voc Day Hab Ongoing family support services **This latter program doesn’t require Medcaid
Life Adjustment Center Openings are in their new Harlem Day Program 3 slots are open for non ambulatory 10 slots are open for low functioning individuals
New York League Openings are in the Day Programs and Pre Vocational Programs Partnership for Comprehensive Care Openings are in the Day Program for non ambulatory clients
Resources For Children Openings are allotted for up to 30 hours of intensive services for those families that reside in Manhattan
YAI Openings are available in their enhanced employment and in their supportive employment
Fairs: District 75 will be hosting its annual Epic Fair on Saturday May 19th from 9-2 The place of fair is still to be determined
Community Habilitation: This is a waiver service in which a direct care worker works with a client in their home or local community The caregiver doesn’t need to be home while this service is going on in the home, however, many parents do choose do be present
Student Information Exchange: This will take place on Wednesday, February 1st, 2012
The information package for each student should contain a narrative summary of the students including strengths and desires as well as an idea of the support/services he/she will be seeking. It should also include the most recent IEP as well as the Psychological, Psychosocial, and Medical evaluation. If the student has MSC, the current ISP would also be helpful. It is important to realize that if you don’t have the whole evaluation packet, you should include what you have and indicate the status of any evaluations that maybe in process. Please keep in mind that the Student Information Exchange is just one activity to assist in the transition process. As students and/or families are looking at and touring the various Adult Day Programs, if they decide on their choice, the referral process can begin at any time. You do not need to wait for the Student Information Exchange to begin the referral process. The sooner the referral process begins, the easier it will be to ensure a smooth transition. If there are any special situations such as the need for more intensive supports or non Medicaid funded programs, there is more of a need to identify potential providers early
