Manhattan Resource Fair:
The fair this year will be on December 1st, 2011 From 10 -2
The location will be St Paul the Apostle Church located at 405 W. 59th Street at 9th Avenue
Volunteers are need for picking up the food at Costcos’ and delivery of the food
to the church
Examples of workshops at the fair are: 1115 Waiver & Travel Training

Open School Dates:
No open school dates were given out at this transitional meeting
However, the Transition Committee is offering to conduct informational presentations for the schools on OPWDD services and Transition Process. These presentations can be geared to either school based staff and/or parents/families. Kathy Kelly will be presenting one of these workshops at the NY Center for Autism Charter School on Nov. 22nd.

Student Information Exchange:
Will be February 1, 2012 at 75 Morton Street

The information package for each student should contain a narrative summary of the student including strengths & desires as well as an idea of the supports/services he/she will be seeking. It should also include the most recent IEP as well as Psychological, PsychoSocial and Medical evaluations. If the student has MSC, the current ISP would also be helpful. It is important to realize that if you don’t have the whole evaluation packet, you should include what you have and indicate the status of any evaluations that may be in process.

Please keep in mind that the Student Information Exchange is just one activity to assist in the Transition process. As students and/or families are looking at and touring the various Adult Day Programs, if they decide on their choice, the referral process can begin at any time. You do not need to wait for the Student Information Exchange to begin the referral process. The sooner the referral to a program begins, the easier it will be to ensure a smooth transition. If there are any special situations such as the need for more intensive supports or non-Medicaid funded programs, there is more of a need to identify potential providers early.

1115 Waiver or People’s First Waiver:
This is the new proposed waiver of OPWDD and is the anticipated direction for future services. At this time, the process is still in an exploratory stage however OPWDD has done significant work on this project over the past several months. Detailed information can be found on OPWDD’s website at www.OPWDD.ny.gov. Generally what this will mean is that services & supports will be managed in a different way. This process is expected to be implemented over the next 5 years. All are encouraged to visit OPW’s website and read up on the information. There is also an opportunity to provide input through scheduled public forums as well as via e-mail through OPW’s webpage.
