It was announced that Kathy Beiser from Esperanza will not be able to co-chair this committee at this time, however Lucy Kramer from Life Adjustment Center has agreed to Co-Chair.

*The Manhattan DD Council’s Website has a new look. The Website is www.Manhattanddcouncil.org. This Website contains the minutes from the Council’s Meetings as well as the minutes from the various Sub-Committees including this Transition Committee. It also contains a variety of directories including Family Support & Adult Day Programs along with other resources & links.

OUTREACH:
The Transition Committee will be available to be present at the Open School events this fall and/or to present at School PTA or Staff meetings to provide information about the Transition Process and OPWDD services. At this time, the schools do not have the dates for the Open Schools.
REVIEW OF ADULT DAY SERVICES DIRECTORY:
Based on a suggestion made at our meeting this past June, the Committee reviewed the format of the Adult Day Services Directory for the purpose of providing more relevant, helpful information to individuals, families and others who use the Directory. Some of the suggestions made include adding information on the size of the program, staff ratios, schedules or hours of operation. Also adding new categories of services including E-SEMP, and a section for Other Options including Specialized Programs and Blended Programs.
A questionnaire will be formulated and sent out to the providers to gather information to put into the updated directory.
