In order to ensure a seamless transition, we are asking for you to please review your rosters carefully and share the following important information with your staff. If there are individuals currently on your roster who are not receiving Medicaid Service Coordination (MSC) or Plan of Care Support Services (PCSS) or to whom you are currently providing MSC or PCSS to that do not wish to continue services when our system transitions to care management with Care Coordination Organizations (CCO) it is important that your agency follows the already existing process for withdrawing the individual from Service Coordination as soon as possible prior to 6/30/2018. This includes individuals that are no longer eligible for services. The service coordinator must complete and submit an Individual Withdraw from MSC Services (MSC3) form in CHOICES and notify the Regional Office’s MSC Regional Coordinator to allow for processing as soon as possible, but prior to 6/30/18 when MSC and PCSS will end. This will provide OPWDD with an accurate record of who will not be moving forward with OPWDD services.
Please ensure that those choosing to withdraw have been informed that, in accordance with OPWDD Home and Community Based Services (HCBS) Waiver requirements, all waiver enrolled individuals must be enrolled in a CCO and has chosen to receive either Health Home Care Management or Basic HCBS Plan Support to receive services on or after 7/1/2018.
