Access to Health and Food Benefits

- Public Health Solutions (PHS) develops, implements, and advocates for solutions to prevent disease and improve community health.
- PHS conducts research on public health issues, create and manage programs, and provides services to organizations to address public health needs.
- Access to Health and Food Benefits (AHFB) is a program dedicated to helping individuals and families access health-related public benefits such as Supplemental Nutrition Assistance Program (SNAP), Health Insurance Navigator Program for New York, and Health Insurance Enrollment for the Aged, Blind, Disabled.
- The Health Insurance Navigator Program helps individuals, families, and small businesses enroll in health insurance through the NY State of Health Marketplace.
- The Navigator Program serves all five boroughs and Long Island.
- This program is for individuals and families under the age of 65.
- Supplemental Nutrition Assistance Program (SNAP) helps families and individuals supplement their diet with nutritious foods by providing financial assistance.
- SNAP at PHS has Benefits Counselors to assist those in need of adequate food to apply for and recertify SNAP benefits.
- SNAP at PHS also provides referrals to other services.
- Facilitated Enrollment for the Aged, Blind, and Disabled (FE-ABD) program provides education, outreach services, and health insurance application assistance to individuals aged 65 or older, certified blind, and certified disabled.
- For access to FE-ABD services, there are Facilitated Enrollers at community-based sites where Aged, Blind, and Disabled individuals obtain services.
- Home visits are conducted to increase accessibility for home-bound aged, blind, and individuals with disabilities.

- Eligibility: Individuals must be a Medicaid beneficiary with a DD, known to OPWDD, living in an IRA, living in their own apartment, living with their families, or transitioning from institutional care.
- Goals include enabling people to return or remain in their community, reduce the number of hospital visits, increase independence, increase quality of life, and open to work with other organizations.
- Technology supports includes Remote Support, Telehealth, Automatic Med Dispenser, and
Assistive Environmental Control.
