Partners Health Plan Presentation
- Partners Health Plan (PHP), a not-for-profit managed care organization that provides Medicaid and Medicare covered supports and services for persons with intellectual and developmental disabilities.
- PHP employs a person-centered approach model to care planning and service delivery that addresses the needs of the whole person.
- To be eligible, you must be 21 years old, have both Medicaid and Medicare, eligible for OPWDD services, and live in NYC, Nassau, Suffolk, Rockland or Westchester Counties.
- You have the benefit of having a single organization manage and incorporate all of the services and supports you receive including medical, dental, pharmacy, behavioral health, OPWDD services and long-term services and supports such as habilitation, occupational therapy, and personal care.
- There is no cost.
- You will have a two-person Care Coordination Team that consists of a Care Manager, a registered nurse who will help manage health care needs and personal goals and a Care Coordinator who will assist with activities.
- An Interdisciplinary Team (IDT) and your provider will meet with you twice a year to help assess your needs and will develop and approve your Life Plan, which is a person-centered plan that includes all of your health, social, behavioral, OPWDD services and long term care services.
- Your IDT will authorize your services except for residential care.
- You won’t have to wait to get through “The Front Door”.
