June 2016 Graduates Update:
At this time the schools project approximately 90 Manhattan students expecting to graduate in June 2016 who will be transitioning into OPWDD Adult Services. The school staff reported that the vast majority have already been determined to meet OPWDD eligibility and now have MSC in place. The school transition representatives are actively working with the Metro DDRO’s staff to coordinate & ensure the
process is underway. All graduates need to go through the Front Door. About 10, need to complete the eligibility determination process, the remaining graduates have begun the Front Door process. The schools have been working with the DDRO to schedule Front Door sessions at some of the school sites.
The schools identified about 8 people where the families have been unresponsive or have not been actively involved in the transition/Front Door Process. The committee discussed ways to assist such as involving the Parent Coordinator at the schools and/or having the student participate as a self-advocate if they are capable of doing so. The committee also strategized on a few more challenging situations.
A request for additional information/guest speaker was made for the topic of Transitioning Students currently in the Bridges 2Health Waiver. We will try to have a speaker address this topic before the end of the year.
Next Meeting: APRIL 13, 2016 9:30am – AHRC, NYC – 83 MAIDEN LANE, New York, NY – 11th Floor
